What's the difference between a porter and a stout?

Porters and stouts seem very similar and sometimes stores and restaurants group them together as if they're two different names for the same thing. Is there a difference and what is it?

Does milk stout actually contain lactose?

I know that lactose is used in the brewing process for milk stout, but I don't know if it remains there by the end of the brewing process. I'm very lactose intolerant, and want to know if I need to take a lactase pill if I drink a milk stout.
